FORM Theme Documentation
Welcome to the documentation for FORM, a premium Shopify theme by Modulus.
FORM is a luxury, conversion-focused theme built around a single hero product, with room to grow into a small catalogue. It pairs an editorial homepage funnel — hero, product spread, buy block, split feature, dotted-leader lists, testimonials, promotional banner, and FAQ — with a full set of storefront templates. The look is a light, paper-toned editorial system with an Anton display face, restrained motion, and a single red accent.
FORM works well for:
- Single-product stores — build a long-scrolling product funnel on the homepage.
- Small-catalogue stores — lead with a featured product, then link out to a collection of accessories or a wider range.
Getting started
Requirements
FORM runs on any Shopify plan. A few optional features rely on free Shopify tools:
- Shopify Search & Discovery app (free, by Shopify) — required for collection filters, variant swatches, and complementary product recommendations.
- A subscriptions app — required only if you want to sell subscriptions through the buy block's Subscription block. The block appears automatically when a product has a selling plan attached.
- Shopify Markets — used for the footer country/language selector.
Nothing else is required. Rich media (video and 3D models), gift cards, and customer accounts all use native Shopify features.
Installation
- In the Shopify admin, go to Online Store → Themes.
- Add FORM to your theme library from the Shopify Theme Store.
- Find FORM under Theme library, click Customize to open the theme editor, or Actions → Publish to make it live.
Demo content does not transfer. The images, text, products, and menus shown in the FORM demo are not included when you install the theme. Everything you see in the demo is reproducible with your own products and the settings documented here.
First things to configure
After installing, open Customize and work through these in order.
1. Brand — logo and favicon
In Theme settings → Brand: upload your logo image, set the logo width (80–260 px), and upload a 32 × 32 px favicon.
2. Colors and scheme
In Theme settings → Colors, review the color schemes. FORM ships with three: a light paper scheme (the base), a dark carbon scheme, and a plain white scheme. Edit them or add new ones, then assign a scheme to each section.
3. Fonts
In Theme settings → Typography, choose your fonts. FORM defaults to Anton (display), Archivo (body), and Archivo (labels).
4. Navigation
Go to Online Store → Navigation to build your header menu (main-menu) and footer menus. The header turns any top-level menu item with sub-items into a mega menu automatically.
5. Your featured product or collection
- For a single-product store, set your product on the Hero and Buy block sections.
- For a collection-led homepage, add a Featured collection section and choose a collection.
6. Cart cross-sell (optional)
In Theme settings → Cart, set a cross-sell heading and up to three cross-sell products.
Choosing your layout
FORM works as a single-product funnel or a collection-led storefront — you decide by which sections you add to the homepage.
Option A — The single-product funnel
Lead with one product and let the page do the selling. This is how the FORM demo is built:
- Hero — full-width banner with your product image, headline, and a button that scrolls to the buy block.
- Product spread — an editorial introduction to the product, image beside notes and a short description.
- Split feature — an image beside a heading, rich text, and a small specification list.
- Buy block — the product's gallery and purchase area, composed from blocks.
- Marquee divider and Dotted leader list — typographic rhythm and scannable detail.
- Testimonials, Comparison, Press logos, Trust badges — social proof.
- FAQ — objection handling.
- Newsletter or Back in stock — capture.
To point the funnel at your product, set the same product on the Hero, Product spread, and Buy block sections. Any section can be hidden with the eye icon without deleting it.
Option B — The collection-led homepage
- Hero — set an image and headline; link the button to your collection.
- Featured collection — choose a collection and show a grid (2–12 products, 2–4 columns).
- Add supporting sections as needed.
Mixing both
A hero and buy block for your flagship, followed by a Featured collection for accessories. The Cart cross-sell and Recommendations section help move shoppers between products.
The product templates
FORM includes three product templates:
- Default product template — buy block, collapsible content, recommendations, and a sticky add-to-cart bar. Good for accessories.
- The "flagship" product template — buy block, macro band, FAQ, recommendations, and sticky bar. A richer, funnel-style page for your hero product.
- The "gift-card" product template — a tailored page for selling a gift card, with a value picker, gift-appropriate reassurance, and a "spend it on" rail.
Assign a template under Products → (your product) → Theme template.
Theme settings
Theme settings are global — open them from Customize → Theme settings (the gear icon).
Brand

Colors
FORM uses Shopify color schemes. Each scheme defines:

Input corner radius — corner rounding for inputs and buttons, 0–10 px (default 3).
FORM ships with three schemes: Paper (light base), Carbon (dark), and White.
Cart

Social media
Add the full URL for each profile. Icons appear in the footer only for fields you fill in. Available: Instagram, Facebook, X, TikTok, YouTube, Pinterest, LinkedIn.
Sections
Header & footer
Announcement bar — A slim message bar above the header. Add one or more Message blocks; multiple messages rotate. Settings: Color scheme · Change every (3–9 s).
Header — Settings: Brand name · Menu · Make header transparent over hero. Top-level menu items with sub-items become a mega menu automatically.
Footer — Settings: Brand name · Tagline · Legal menu · Show payment icons. Up to 4 Link column blocks (heading + menu). Country/language selector appears automatically when more than one market or language is configured.
Promotional popup — Email-capture popup, off by default. Settings: Enable popup · Color scheme · Eyebrow · Heading · Body · Button label · Image · Discount code · Success message · Open trigger (delay or exit intent) · Delay (2–30 s) · Show again after (1–60 days).
Homepage & product sections
Hero — Full-width banner. Settings: Color scheme · Hero layout (Inset image / Statement overlay) · Image · Eyebrow · Heading · Outline line (which heading line draws as outline, 0 = none) · Product · Button label · Vertical edge label · Scroll label · Text protection over image (0–70%).
Buy block — The gallery plus purchase area, composed from reorderable blocks:

Product spread — Editorial introduction: image beside notes, a short description, a spec line, and a button. Settings: Product · Color scheme · Eyebrow · Notes · Description · Value (spec line) · Button label · Image · Caption · Flip image to the right.
Split feature — Large image beside a heading, rich text, optional button, and a small specification list. Settings: Color scheme · Image · Caption · Flip image to the right · Eyebrow · Heading · Text · Button label · Link. Each Specification block: Label · Value.
Marquee divider — Scrolling band of short phrases used as a typographic divider. Settings: Color scheme · Loop duration (20–90 s; pauses with reduced motion). Each Phrase block: Text · Draw as outline type.
Dotted leader list — List of dotted-leader rows (label left, value right), optionally numbered. Settings: Color scheme · Eyebrow · Heading · Show index numerals. Each Row block: Label · Value · Text · Link.
Macro band — Full-width band showing a zoomed, focal-point crop of an image behind an outlined heading. Used on the flagship product template. Settings: Color scheme · Heading · Image · Zoom (1–5×) · Horizontal/Vertical position · Minimum height (280–720 px).
Image hotspots — An image with positioned hotspot markers that reveal detail. Each hotspot: Horizontal/Vertical position (0–100%) · Title · Text.
Testimonials — Customer quotes. Each block: Quote · Name · Role.
Comparison — Comparison table between your product and an alternative. Each row: Feature · Your product value · Comparison value.
Promotional banner — Promo strip with heading, text, and up to two buttons. Optional background image with darkening overlay. Settings: Color scheme · Eyebrow · Heading · Text · Button label · Link · Secondary button · Alignment (Left/Center) · Minimum height (280–720 px) · Image · Image overlay (0–80%).
Press logos — A row of publication logos. Each block: Name · Logo image · Link.
Trust badges — A row of reassurance badges. Each block: Title · Text.
FAQ — Expandable question-and-answer list.
Newsletter — Email signup with a reveal-a-code reward. Create a matching discount in Settings → Discounts; the code is revealed after signup.
Back in stock — Email-capture for a sold-out product.
Featured collection — Grid of products from one collection. Settings: Collection · Products to show (2–12) · Columns on desktop (2–4) · Show 'view all' link.
Video — Native or hosted video with a poster image. Settings: Video · Poster image · Orientation · Autoplay (muted loop).
Collapsible content — Accordion or tabbed panels for shipping, returns, care, etc. (Product template.)
Recommendations — Related or complementary products. Settings: Recommendation type (Automatic / Manual) · Recommendation intent (Related / Complementary) · Products to show (2–8). (Product template.)
Sticky add-to-cart bar — A bar that follows the shopper down the page with price and add-to-cart.
Custom section — A flexible section built from theme and app blocks.
Custom Liquid — Add your own Liquid, HTML, or app snippet as a standalone section.

Assign a template to a product under Products → (your product) → Theme template.
Guides
Setting up the single-product funnel
- Add your product in Products (images, description, price, and variants).
- On the Hero section, set your product, image, headline, and button label.
- On the Product spread section, set the same product and fill in the notes, description, and spec line.
- On the Buy block section, set the same product. Add, remove, and reorder blocks until the purchase area reads the way you want.
- Fill in the supporting sections — Split feature, Dotted leader list, Testimonials, Comparison, FAQ — or hide the ones you don't need.
- Set your product on the Sticky add-to-cart bar.
Color schemes
- Go to Theme settings → Colors.
- Edit the existing scheme. Solid buttons use Accent solid; links use Accent bright; Accent carries large display text and decorative marks.
- To offer more than one look, add a scheme and set its colors.
- Assign a scheme per section: open any section with a Color scheme setting and pick the scheme you want.
Variant swatches
- Install the free Shopify Search & Discovery app.
- In the admin, set up swatches for your product option by assigning colors or images to each value.
- The Variant picker renders those swatches automatically.
Mega menu setup
- Go to Online Store → Navigation and open your header menu.
- Add a top-level item and sub-items beneath it.
- Any top-level item with sub-items becomes a mega menu panel automatically.
- The panel's featured image is pulled from the linked item (a collection's image, or its first product's image).
Selling plans & subscriptions
- Install a subscriptions app that creates Shopify selling plans.
- Attach a selling plan to your product.
- The Subscription block in the buy block appears automatically for that product.
Faceted filters
- Install the free Shopify Search & Discovery app.
- In the app, add the filters you want (availability, price, product type, options).
- On the Collection section, make sure Enable filtering is on.
Rich product media (video & 3D)
In Products → Media, add video or a 3D model alongside your images. The gallery plays them when selected. For a standalone video, use the Video section.
Performance tips
- Upload appropriately sized images; avoid enormous files.
- Use a single strong hero image above the fold.
- Add video through Shopify-hosted media where possible rather than heavy third-party players.
- Keep the number of apps that inject scripts to a minimum.
- Prefer the theme's built-in sections over large blocks of Custom Liquid.
FAQ
Can I use FORM with only one product? Yes — that is its core use case. Build a single-product funnel on the homepage with the Hero and Buy block, then add supporting sections.
Can I use FORM with several products? Yes. Add a Featured collection section to the homepage, use the Collection template for browsing, and give products different templates.
How do I change my store's colors? Go to Theme settings → Colors. Solid buttons use Accent solid and links use Accent bright, so changing those restyles them everywhere.
How do I upload my logo? Theme settings → Brand → Logo image, then set Logo width. If empty, the header shows the Brand name text instead.
What fonts can I change? Three: Display and headings, Body, and Labels and specifications, all in Theme settings → Typography. Defaults are Anton, Archivo, and Archivo.
How do I set up collection filters? Install Shopify's free Search & Discovery app, add the filters you want, and make sure Enable filtering is on for the Collection section.
How do I edit the navigation and mega menu? Menus are managed in Online Store → Navigation, not the theme. Any top-level item with sub-items becomes a mega menu automatically.
Does FORM support subscriptions? Yes, through the buy block's Subscription block. Install a subscriptions app that creates Shopify selling plans and attach a plan to your product; the block appears automatically.
Can I sell gift cards? Yes. Use the gift-card product template for the sales page, and the gift card template renders automatically for issued cards — with the code, a QR code, an Add to Apple Wallet button, and your logo.
How do I add video or a 3D model to a product? Add them in Products → Media alongside your images; the buy block gallery plays them.
How do I add a currency or language selector? Configure markets and languages with Shopify Markets in the admin. FORM shows a footer selector automatically when more than one option exists.
The demo content didn't appear after install — is that a bug? No. Shopify never copies demo content to your store on install. Every demo layout is reproducible with your own products and the settings in this documentation.

Support covers the FORM theme itself. Setup of third-party apps, custom code, and store-wide Shopify configuration fall outside theme support, though we'll always point you in the right direction.
